Next, you'll need to head to Apple's $100 early iPhone buyer store credit page and enter your phone's serial and mobile phone numbers.

Subsequently, you'll receive an SMS message on your iPhone that provides an access code you can use to redeem your credit. In the fields provided, enter the access code to generate the store credit.

That's it. You'll receive a $100 store credit good towards purchases at the Apple Store.
